I had a medical examination for my driving licence C.

I did not receive a vAT receipt for this. Is it still possible to include this cost in accounts (I have a piece of paper stating the amount, but it is not an official invoice).

Yes, it will still work if you have a document from that party stating what they did and what it cost, you can still include it in your accounts. VAT recovery (if there is VAT on it, no idea) will not work, but you can certainly include the cost.
